To assist our clients in complying with 29 CFR 1904 OSHA's Record Keeping Rule for Injury & Illness reporting, we have provided an AmStaff Injury & Illness Report form.  Proper completion of our form for any work-related injury or illness will satisfy all the data entry requirements of the OSHA 301 form, provide the AmStaff Risk Management Department with the required information to correctly complete the OSHA 300 that we maintain for our clients and provide the AmStaff Risk Management Department with valuable information to better serve our clients in reducing workplace injuries.

The following are excerpts from OSHA's Record Keeping Rule:

"This Injury and Illness Incident Report (OHSA 301) is one of the first forms you must fill out when a recordable work-related injury or illness has occurred.  Together with the Log of Work-Related injuries and Illnesses (OSHA 300) and the accompanying Summary (OSHA 300A), these forms help the employer and OSHA develop a picture of the extent and severity of work-related incidents."

"Within 7 calendar days after you receive information that a recordable work-related injury or illness has occurred, you must fill out this form (OSHA 301) or an equivalent.  Some state workers' compensation, insurance, or other reports may be acceptable substitutes.  To be considered an equivalent form, any substitute must contain all the information asked for on this form (OSHA 301)."

"According to Public Law 91-596 and 29 CFR 1904, OSHA's recordkeeping rule, you must keep this form (OSHA 301) on file for 5 years following the year to which it pertains."
