Landrum Professional offers a wide variety of benefits for its employees.  Information and FAQs about our benefits are summarized below.  If you need additional assistance, please feel free to contact our Benefits Department.

Health
Health insurance varies by client agreement, contact the Benefits Department for information.

Dental 
Dental insurance is provided through Guardian.   Employee participation levels are not mandatory, so any number of employees at a client location may participate.  Preventative maintenance is paid 100%.  Maximum coverage per year is $1,000 per person covered.

Vision
Vision insurance is provided through Vision Care Plan.  Our vision plan provides eye exams, glasses and contacts with a small co-pay when services are performed by participating optometrists.

Supplemental Insurance
Disability Income Protection Insurance, long term care, term life, universal life, cancer and accident policies are available through selected agents at discounted rates.  Premiums can be payroll deducted.  Rates vary depending on income job classification, age and amount of coverage desired.  Please contact Ron Sedlacek at Sedlacek Insurance Service at (850)477-8086 for more information.

Benefit Expense Account Plan (Cafeteria Plan)
Landrum Professional offers all of its clients and their employees a premium only plan that allows employees to pay for insurance premiums with pre-tax dollars. This saves the employees money every pay day that would normally be deducted for taxes. There is no charge for this benefit.

Employee Assistance Program - List of Providers
After much research, we have chosen to use Behavioral Health Systems (BHS) for our Employee Assistance Program.   BHS has a nationwide provider network which will provide a higher level of satisfaction for our employees and clients. Services cover a wide range of problem areas including: stress, marital, family, psychiatric and substance abuse.

Benefits may be accessed by calling 800-245-1150, 24 hours a day, 7 days a week.  BHS also has an on-line service available for resources and reference information.  Visit the website at www.behavioralhealthsystems.com.  Our password is LANDRUM.

Cordova Counseling is still an EAP provider, now through BHS. Call the BHS toll free number for authorization.

Tuition Reimbursement
Employees who have completed one year of full time service with Landrum Professional are eligible to receive reimbursement of 50% of college tuition up to $250 per year. Courses taken must be job related and approved in advance.  Courses must also be taken at an accredited institution and college credit must be granted.

Adoption Assistance
Employees who have completed one year of full-time service with Landrum Professional, and who adopt a child through a state-approved agency, may be reimbursed up to $500 for legal fees incurred in that adoption. Step-children do not qualify.

Credit Assistance
Allvista Solutions, Inc. is a non-profit organization that provides financial counseling for families and individuals with serious debt problems. A counselor will review your financial condition with you, help you design a manageable budget to suit your needs, help you put your budget to work, and monitor your progress recovery.

Frequently Asked Questions Regarding Benefits

When are my benefits effective?

Once you have worked for three (3) months full time (30 or more hours each week), you are eligible the first of the next month following those three months.  Example 1: If your date of hire is October 12, your coverage would begin February 1st.  Example 2: If your date of hire is October 1, your coverage would begin January 1st.
When do the deductions start? 
The deductions will start one month prior to your insurance effective date.  Example:  If your insurance starts February 1, your deductions will start
January 1.

Can I add my family to my insurance?   
There are two times, other than the original enrollment, that you can add family members:  
1)  During open enrollment of the plan, which is once per year.
2)  You have special enrollment rights.  Life changing events such as marriage, birth, involuntary loss of coverage, and adoption all allow for additions within 30 days of the event.  If the 30 days pass, you would have to wait until open enrollment to add to your policy.  Click here for the Member Status Change form to add members to your policy.    

Can I cancel family members from health (and/or dental) insurance? 
Cancellations can be made anytime for those NOT on the cafeteria plan. Those not on the cafeteria plan can cancel family members for the end of the month in which we receive the request.  The IRS regulations for the cafeteria plan allow for cancellations at the end of the cafeteria plan year December 31st or at the open enrollment of the plan.   Click here for the Member Status Change form to cancel family members from your policy.

Can I insure my fiancé or “significant other”? 
No.  Dependents on your plan must be your legal dependents.  A present spouse, unmarried natural child, adopted, foster, or step child(ren) or a child for whom the insured has been court-appointed as legal guardian or legal custodian who is under the age of 19 are eligible.  Upon marriage, you have 30 days to add your new spouse to your plans.  Common law spouses are not eligible for coverage.

Which dentists can I go to on the dental plan?
 Any dentist.
How do I use my vision benefit?  You first request a voucher and take it with you to one of the participating providers.  The vouchers can be ordered by calling toll free 1-800-865-3676 or on-line at www.visioncare.com.  The voucher is mailed to your home along with a list of providers in your state.  You will need to take this voucher with you to your appointment.  It is good for two months.  If you do not use it in that time, you can return it to Vision Care Plan and order another one.

I am interested in other insurance such as short term disability or cancer coverage.  Can I get them through Landrum Professional?
These types of coverage are available through payroll deduction.  Allstate Insurance is the carrier.  Because rates vary based on age and income (for disability) and the amount of benefit requested (for life insurance), please contact Ron Sedlacek at Sedlacek Insurance Service at (850)477-8086 for rates.
